| December work hours were sent to the Western Washington office the last week of January and have been converted to the new Dollar Bank system. the Trust office plans to mail benefit statements to all active members by the middle of the month showing future eligibility under the new system. | |

Enrollment Documentation |

| The trustees for the merged
health plan are requiring marriage and dependent documentation along
with new enrollment forms. In order to continue processing claims, the
documents need to be sent in by the end of February. Photo copies of
certified documents will work, You do not need to send originals. The
required documentation for Washington- Idaho plan member families is the
same as for all newly eligible participants.

The newsletter also describes plan programs for addressing chronic illness and smoking cessation, and other health information resources. These newsletters often contain plan information that is benefit and time sensitive. Members are strongly encouraged to read these newsletters. | |
